Community Programs & Participation

  • Every individual and family impacted by CP, regardless of SDH barriers, will have access to a CP-specific overarching community programs that provide children & families with continuity of care across the childhood years that connects children & families to local resources, referrals to continued care, current research initiatives.​

  • Every community, regardless of region of GA and SDH barriers, will have access to comprehensive CP support.​

  • Community stakeholder education on available resources and how to access them.​

Clinical Excellence & Lifetime Care Transitions

  • Every individual impacted by CP will have multidisciplinary, ICF informed care plan that encompasses CP complexities and transitions care across the lifespan ​ 

  • Every provider willing to participate in CP care will have a practice plan, including billing code and research-informed specialized training, for taking on complex care patients​

  • Best clinical practice evidence for CP will be incorporated into care provider pipelines and environments.

Research & Technology

  • Every individual will have access to lay-person friendly CP research findings and will have the opportunity to help to inform research questions surrounding CP.

  • Every researcher-scientist will have access to a research network in GA that will identify common research priorities, guide community-informed clinical investigation and research, assist with recruitment, and promote dissemination to lay and professional audiences​.

  • Build family capacity to participate in design and understanding of research.

Stakeholder Engagement & Education Pipelines

  • Every medical and allied health professional who contributes to CP care and education will be integrated into a professional training resources/program that integrates evidence-based outcomes and interventions.​

  • Every medical and allied health student with the desire to participate in CP care will have access to CP Specialty certifications to incentivize exemplary knowledge and commitment to individuals with CP to strengthen the pipeline of skilled providers ​

  • Every academic institution will have access to tools for supporting inclusion and accommodation specific to physical disability​
